Since the 1999 release of the Institute of Medicine’s report, To Err is Human: Building a Safer Health System, safe medical care has been spotlighted as a priority in the nation’s healthcare systems. Even before that, Pulse Center for Patient Safety Education & Advocacy worked to improve patient outcomes.

Tejal Gandhi, MD, MPH, CPPS

Chief Safety and Transformation Officer, Press Ganey

About Dr. Gandhi

Tejal Gandhi, MD, MPH, CPPS, is the Chief Safety and Transformation Officer at Press Ganey. In this role, she is responsible for advancing the Zero Harm movement, improving patient and workforce safety, and developing innovative health care transformation strategies. In addition, Dr. Gandhi is leading the Press Ganey Equity Partnership to advance equity in health care.

Before joining Press Ganey, Dr. Gandhi served as Chief Clinical and Safety Officer at the Institute for Healthcare Improvement (IHI), where she led IHI programs focusing on improving patient and workforce safety. Prior to this, Dr. Gandhi was President and Chief Executive Officer of the National Patient Safety Foundation (NPSF) from 2013 until 2017, when NPSF merged with IHI. She has also served as President of the Lucian Leape Institute, a think tank founded by NPSF that now operates within the IHI Safety portfolio and was a board member of the Certification Board for Professionals in Patient Safety which administers the Certified Professional in Patient Safety credential.

Helen Haskell – Co-chair

Since the medical error death of her young son Lewis in 2000, Helen Haskell has worked to bring the patient voice to healthcare safety and quality. She is president of the nonprofit patient organizations Mothers Against Medical Error and Consumers Advancing Patient Safety and a board member of Patient Safety Action Network and the International Society for Rapid Response Systems. She is former co-chair of the WHO Patients for Patient Safety Advisory Group, and a senior fellow of the Institute for Healthcare Improvement. She has published over 50 articles and book chapters and is co-editor of an interprofessional textbook using patient narrative to teach patient safety and professional competencies.

Robin Moulder, MBA, RN, BSN, CPHQ – Co-chair

Robin Moulder is a Distinguished Advisor to Pulse Center for Patient Safety Education & Advocacy. She is Senior Manager in the Division of Quality & Safety, Memorial Sloan Kettering Cancer Center (MSKCC) in New York and has served in nursing for over 30 years. She began her career in acute care then transitioned into home infusion therapy, holding various clinical coordination and administrative positions in quality & staff education. She volunteered for 7 years as Co-Chair of the MSKCC Patient Safety Advisory Council , then in 2012, joined MSKCC’s team full time. Robin has been credentialed as a CPHQ for over 20 years and is a certified Six Sigma Green Belt.
